- Single Residential Lots: MT-EZ
Conditional Letter of Map Amendment (CLOMA)
- Single Residential lots, properties containing commercial structures, or multiple lots (Residential or Commercial): MT-1
Letter of Map Revision (LOMR)
- Single or Multiple lots (Residential or Commercial): MT-2
Conditional Letter of Map Revision (CLOMR)
- Single or Multiple lots (Residential or Commercial): MT-2
Letter of Map Revision - based on fill (LOMR-F)
- Single Residential lot or structure, property containing commercial structures, or multiple lots (Residential or Commercial): MT-1

Community Acknowledgement for map amendments and revisions
Submit the Original and One Copy of the appropriate FEMA Document and required attachments to:
All original documents will be returned to the applicant who is responsible for forwarding the request to the Federal Emergency Management Agency and paying all associated fees and costs.

Letter of Determination Review (LODR)
A letter of determination is the procedure used by property owners who disagree with their lenders determination that they are in the floodplain. The request is time sensitive and there is a processing fee. Specific program information and forms are available from the following links.
